About The Position

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in private residences and senior living communities in Trenton, New Jersey, and surrounding communities. Using the Geriatric House Calls™ model, clinicians help patients improve mobility, independence, wellness, and quality of life. The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and incorporate telehealth or other virtual care when appropriate. FOX offers full-time, part-time, and PRN/FOX Flex employment pathways. The stated standard annual base pay range for a full-time role in New Jersey is $70,000–$102,000, with the opportunity to earn more; part-time and PRN compensation is prorated.

FOX Rehabilitation is a clinician-operated practice focused on meaningful relationships, evidence-based care, and a mission to abolish ageism. Clinicians work with an interdisciplinary team and may have opportunities to earn additional income through referral bonuses and incentives, clinical instruction, mentoring, supervising therapy assistants, care above bi-weekly quotas, and creating content. Eligible roles include benefits and professional development support.

Requirements

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
• Ability to wear PPE when required.

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
• Document care in FOX’s EMR system in accordance with regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth or virtual care when appropriate.
• Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
