Physical Therapist - Piscataway, NJ
About The Position
FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in private residences and senior living communities in and around Piscataway, New Jersey. Clinicians use the Geriatric House Calls™ model to help patients improve mobility, independence, wellness, longevity, and quality of life. The role offers autonomy to create and manage a treatment schedule and build lasting patient relationships. Clinicians may also incorporate telehealth and other virtual care into their schedules. The position is available as full-time, part-time, or PRN/FOX Flex. Full-time roles are salaried with benefits; part-time roles are salaried with benefits eligibility based on a minimum-hours requirement; PRN/FOX Flex roles are compensated per unit. New graduates are eligible for FOX’s Emerging Professionals Mentor Program, and H1B sponsorship is available for qualified clinicians.
Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, CPR, patient transfers and bed mobility, repositioning patients, extended standing and walking, climbing stairs, and therapeutic activities.
- Ability to drive to patient homes and senior living communities.
- Wear PPE when required.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Provide therapy to support patient wellness, longevity, mobility, and functional independence.
- Document effectively in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
- Potential opportunities include supervising PTAs/OTAs, mentoring emerging professionals, teaching as a Clinical Instructor, and contributing blog or social media content.
