Physical Therapist - Branchburg, NJ
About The Position
FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in Branchburg, New Jersey, and surrounding communities. Clinicians treat patients in private residences and senior living settings, including independent living, assisted living, and memory care, using FOX’s Geriatric House Calls™ model. The role focuses on improving mobility, independence, wellness, longevity, and quality of life through individualized, evidence-based treatment. Clinicians may incorporate telehealth and other virtual care into their schedules. The position offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and work in a collaborative, interdisciplinary community-based practice. FOX offers full-time, part-time, and PRN/FOX Flex pathways. Full-time roles are salaried with benefits; part-time benefits eligibility depends on a minimum-hours requirement; PRN/FOX Flex compensation is per unit. New graduates are eligible for FOX’s Emerging Professionals Mentor Program, and H1B sponsorship is available for qualified clinicians. The standard New Jersey base pay range for a full-time role is $70,000–$102,000 annually, with an opportunity to earn more. Part-time and FOX Flex compensation is prorated. Additional earning opportunities include referral incentives, serving as a clinical instructor or mentor, supervising assistants, providing care above biweekly quota, and creating written or social media content.
Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
- Ability to drive to patient homes and senior living communities.
- Wear PPE when required.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
- Potentially supervise PTA/OTAs, mentor emerging professionals, teach as a clinical instructor, or contribute written or social media content for additional compensation.
