About The Position

Concept Rehab is seeking a PRN Physical Therapist to join their Post-Acute Rehab team in a Skilled Nursing Facility (SNF). This role involves evaluating and treating patients with mobility, function, strength, and positioning needs. The Physical Therapist will collaborate with interdisciplinary teams, deliver high-quality, patient-centered care, and adhere to APTA standards and state licensure requirements. Requirements

• Bachelor’s, Master’s, or Doctorate in Physical Therapy
• Active Physical Therapist license (state-specific)
• Strong communication and relationship-building skills
• Clinical judgment and problem-solving ability
• Commitment to professionalism, ethics, and confidentiality
• Passion for delivering compassionate, high-quality care

Responsibilities

• Evaluate patients within 24–48 hours of physician orders
• Develop and implement individualized treatment plans
• Supervise PTAs and support care delivery
• Communicate with physicians, care teams, patients, and families
• Document care, progress, and billing accurately
• Participate in care planning and discharge processes
• Provide staff education and training
• Recommend equipment and support patient outcomes
