About The Position

Interim HealthCare of Central MT is seeking a home health Physical Therapist to provide personalized, home-based care to patients in Montana. The role supports patients who have lost mobility, strength, or functionality due to illness or injury, helping them work toward their care-plan goals and maintain independence, dignity, and respect. The therapist works as part of a home health team and educates patients and families about care plans, therapy goals, exercises, self-care, and safe assistance. The position offers flexible assignments and scheduling, autonomy, and a 1:1 therapist-to-patient ratio. This is a PRN/part-time opportunity. A pre-employment 10-panel drug screen includes marijuana; exceptions for medicinal use may be granted with a doctor's order.

Requirements

Graduate of an accredited Physical Therapist program.
Active Physical Therapist license in Montana.
At least two years of physical therapy experience; home healthcare experience is preferred.
CPR certification.
Hepatitis B and TB immunizations.
Knowledge of state and federal home health regulations.
Good clinical judgment, strong interpersonal skills, dexterity, and compassion.
Must pass a 10-panel pre-employment drug screen that includes marijuana; an exception may be granted for medicinal use with a doctor's order.

Responsibilities

Provide home-based physical therapy to patients who have lost mobility, strength, or functionality due to illness or injury.
Work with a home health team that may include an RN, LPN, CNA, HHA, OT, and SLP, focusing on the patient’s plan of care and goals.
Educate patients about their plan of care, established goals, exercises, and self-care.
Coach family members on the plan of care, therapy goals, and safely assisting patients with exercises.
