About The Position
VitalCaring is hiring a PRN Home Health Physical Therapist to provide patient-centered care in patients’ homes. The role focuses on helping patients recover safely, regain mobility, maintain independence, and improve their quality of life. It is autonomous and relationship-driven, with the therapist working closely with patients, caregivers, physicians, and an interdisciplinary care team.
Requirements
- Graduate of an accredited Physical Therapy program with a DPT or Master’s degree.
- Current Physical Therapist license in the state of practice.
- Minimum 1–2 years of experience.
- Strong clinical assessment and critical-thinking skills.
- Excellent communication and patient education abilities.
- Current CPR certification.
- Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities
- Evaluate patients’ functional status, mobility, strength, and rehabilitation potential.
- Develop and implement individualized care plans in collaboration with physicians and the care team.
- Provide hands-on therapy to improve mobility, strength, balance, and independence.
- Educate patients and caregivers about treatment plans, exercises, and safety strategies.
- Monitor patient progress and adjust treatment plans as needed.
- Support safe transitions of care and contribute to discharge planning.
- Identify additional patient needs and collaborate with interdisciplinary teams.
- Coordinate care with nurses, therapists, and physicians.
- Document patient care and progress accurately and promptly in the EMR.
- Participate in case conferences and team meetings.
- Supervise Physical Therapist Assistants in accordance with state guidelines.
- Guide and educate caregivers and support staff.
