About The Position

Discovery at Home is seeking a Physical Therapist in Port Charlotte, FL. This position offers a competitive hourly rate and is available for full-time employment (40 hours per week) or PRN. The Physical Therapist will primarily work within assisted and independent living communities, with some home visits possible. The patient population is primarily geriatric, with cases under Medicare Part A Home Health and some Part B. New graduates will be supported and mentored. The company is open to candidates requiring sponsorship.

Requirements

Graduation as a Doctor of Physical Therapy (or equivalent standing).
State license as a Physical Therapist (eligible for FL state licensure).
Valid driver’s license and reliable transportation.
Current CPR certification.
Completion of a Level 2 background screening in accordance with Florida AHCA requirements (including fingerprinting and background screening through the AHCA Clearinghouse).
Open to candidates who require sponsorship to work in the United States.

Nice To Haves

Previous experience in home health is preferred.

Responsibilities

Provide physical therapy services primarily within assisted and independent living communities.
Conduct home visits within the community as needed.
Manage a caseload of primarily geriatric patients, including those under Medicare Part A Home Health and Part B.
Determine own caseload and work schedule within flexible hours.
Hone skills with a supportive team and achieve excellent work/life balance.
