About The Position

This full-time Physical Therapist position is part of the Greater Charlotte Market Float team, serving pediatric outpatient rehabilitation clinics in Huntersville, Davidson, and University. The role involves evaluating patients, developing and implementing individualized care plans, and documenting progress. The therapist will also provide supervision to PTAs and Rehab Techs, and communicate with physicians and other healthcare professionals. This position requires a Bachelor's degree in Physical Therapy, a valid state license, and BLS certification.

Requirements

Bachelor's degree in Physical Therapy program required.
Applicable state license for Physical Therapy required.
BLS required per policy guidelines.
Outpatient Pediatric Physical Therapy experience needed.

Responsibilities

Evaluates patients, selects and administers assessment techniques, and synthesizes information to establish an appropriate patient care plan.
Develops care plans including necessary modalities, procedures, exercise programs, therapeutic groups, and patient education.
Gathers and interprets appropriate data from the medical record, physician reports, patient, and/or family.
Establishes functional goals in conjunction with the patient and provides evidence-based intervention.
Documents evaluations, tests, progress notes, daily treatment notes, attendance records, patient/family education, outcomes, discharge notes, and charges.
Communicates patient's evaluation and subsequent progress to physician, referral sources, and other clinical staff.
Coordinates patient's schedule and treatment programs, and discharge planning.
Provides supervision of PTA's and Rehab Tech/Aide as required for licensure.
