About The Position

This is a full-time Pelvic Physical Therapist position that will serve as a regional float, covering various outpatient rehabilitation sites in the Northeastern Greater Charlotte Physical Therapy market. The role requires cross-training to multiple sites, including University City Adults, Cabarrus Gateway, Kannapolis area, and Prosperity Crossing. The therapist will be responsible for evaluating patients, developing and implementing individualized care plans, and documenting all aspects of patient treatment and progress. This role also involves supervising Physical Therapist Assistants and Rehab Techs/Aides.

Requirements

  • Bachelor's degree in Physical Therapy program required.
  • Applicable state license for Physical Therapy required.
  • BLS required per policy guidelines.
  • Pelvic Physical Therapy experience needed.

Responsibilities

  • Evaluates patients, selects and administers assessment techniques, and synthesizes information to establish an appropriate patient care plan.
  • Develops patient care plans to include necessary modalities, procedures, exercise programs, therapeutic groups, and patient education based on a holistic view of the patient's diagnosis, condition, age, and discharge needs.
  • Gathers and interprets appropriate data from the medical record, physician reports, patient, and/or family.
  • Establishes functional goals in conjunction with the patient and provides evidence-based intervention to work towards meeting these goals, with ongoing assessment to adjust the plan of care as needed.
  • Documents evaluations, tests, progress notes, daily treatment notes, attendance records, patient/family education, outcomes, discharge notes, and charges as needed.
  • Communicates patient's evaluation and subsequent progress to physician, referral sources, and other clinical staff.
  • Coordinates patient's schedule and treatment programs, and discharge planning.
  • Provides supervision of PTA's and Rehab Tech/Aide as required for licensure.
