About The Position
Always Compassionate Health is seeking a compassionate, reliable, and dedicated Physical Therapist to provide high-quality rehabilitation services to patients in their homes in Nassau County, Long Island, New York. The role offers flexible per diem scheduling and competitive compensation, with an opportunity to help patients regain mobility, restore function, and improve their quality of life. The Physical Therapist evaluates patients, develops and delivers individualized treatment plans, monitors progress, educates patients and caregivers, documents care, and collaborates with other healthcare professionals. The position is classified as a 1099 independent contractor.
Requirements
- Active New York State Physical Therapist license.
- Degree from an accredited Physical Therapy program.
- Strong clinical assessment and rehabilitation skills.
- Excellent communication and organizational skills.
- Ability to work independently and collaboratively.
- Commitment to compassionate, patient-centered care.

- Previous physical therapy experience preferred.
Responsibilities
- Conduct physical therapy evaluations to assess mobility, strength, balance, and functional limitations.
- Develop and implement individualized treatment plans based on patients’ needs and goals.
- Provide therapeutic exercises, gait training, balance training, and mobility interventions.
- Monitor patient progress and adjust treatment plans as needed.
- Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility.
- Maintain accurate and timely clinical documentation.
- Collaborate with physicians, caregivers, and other healthcare professionals to coordinate patient care.
