About The Position

Always Compassionate Health is seeking compassionate, reliable, and dedicated Physical Therapists to provide high-quality rehabilitation services to patients in their homes in Nassau County, Long Island, New York. This is a flexible per diem opportunity as a 1099 independent contractor, with immediate opportunities. Physical Therapists help patients regain mobility, restore function, and improve their quality of life through individualized care.

Requirements

• Active New York State Physical Therapist license.
• Degree from an accredited Physical Therapy program.
• Strong clinical assessment and rehabilitation skills.
• Excellent communication and organizational skills.
• Ability to work independently and collaboratively.
• Commitment to compassionate, patient-centered care.

Nice To Haves

• Previous physical therapy experience preferred.

Responsibilities

• Conduct physical therapy evaluations to assess mobility, strength, balance, and functional limitations.
• Develop and implement individualized treatment plans based on patients’ needs and goals.
• Provide therapeutic exercises, gait training, balance training, and mobility interventions.
• Monitor patient progress and adjust treatment plans as needed.
• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility.
• Maintain accurate and timely clinical documentation.
• Collaborate with physicians, caregivers, and other healthcare professionals to coordinate patient care.
