About The Position

We are seeking an experienced Physical Therapist (PT) – Skilled Nursing Facility (SNF) for a 12-week contract position in Gloversville, New York. The Physical Therapist will provide rehabilitation services to nursing home residents, focusing on mobility, strength, balance, functional independence, and safe progression toward individualized therapy goals. Candidates must have previous Skilled Nursing Facility experience and hold an active New York Physical Therapist license.

Requirements

• Active New York Physical Therapist (PT) License Required.
• Prior Skilled Nursing Facility (SNF) Experience Required.
• Current BLS/BCLS Certification Required.
• Ability to Work Monday–Friday, 8:00 AM–4:30 PM Required.
• Strong Patient Assessment, Rehabilitation Planning, Documentation, and Interdisciplinary Collaboration Skills Required.

Responsibilities

• Evaluate patients and develop individualized physical therapy plans based on functional limitations and rehabilitation goals.
• Provide skilled therapeutic interventions to improve mobility, strength, balance, endurance, and functional independence.
• Monitor patient progress and adjust treatment plans based on clinical response and established goals.
• Collaborate with nursing, rehabilitation, and interdisciplinary healthcare professionals to support coordinated patient care.
• Maintain accurate and timely clinical documentation in accordance with therapy, facility, and regulatory standards.
• Provide patient-centered rehabilitation services designed to improve mobility, function, safety, and independence.
