Physical Therapist - Ahwatukee Post Acute
About The Position
Ahwatukee Post Acute is growing and seeking a dedicated Physical Therapist to join their in-house therapy team. The facility is a 192-bed skilled nursing facility undergoing a major renovation, including a state-of-the-art 44-bed behavioral and dementia care unit. They maintain a CMS 5-Star Quality Measure rating and foster a supportive, collaborative clinical environment focused on patient-centered success. The role involves performing skilled PT evaluations, providing evidence-based treatment, developing individualized care plans, documenting patient progress, and educating patients, families, and caregivers on therapy strategies. The team includes other PTs, PTAs, OTs, COTAs, and an onsite SLP, with no contracting or travel required.
Requirements
- Arizona PT license (or eligible prior to start date)
- Strong communication and teamwork skills
- A passion for working with the geriatric and memory care population

Responsibilities
- Perform skilled PT evaluations and provide evidence-based treatment

- Document patient progress using Net Health
- Collaborate closely with nursing and IDT using PointClickCare
- Educate patients, families, and caregivers on therapy strategies
