About The Position

Ahwatukee Post Acute is expanding and seeking a dedicated Physical Therapist to join its in-house therapy team. The 192-bed facility is currently undergoing a major renovation, including the construction of a state-of-the-art 44-bed behavioral and dementia care unit, which will enhance its ability to provide specialized services to the Phoenix and Ahwatukee communities. The facility proudly maintains a CMS 5-Star Quality Measure rating and fosters a supportive, collaborative clinical environment focused on patient-centered success. The role involves working with a strong and fully staffed in-house therapy department, with no contracting or travel required. Requirements

  • Arizona PT license (or eligible prior to start date)
  • Strong communication and teamwork skills
  • A passion for working with the geriatric and memory care population

Responsibilities

  • Perform skilled PT evaluations and provide evidence-based treatment
  • Develop individualized care plans to increase function, safety, and mobility
  • Document patient progress using Net Health
  • Collaborate closely with nursing and IDT using PointClickCare
  • Educate patients, families, and caregivers on therapy strategies
