About The Position

BAYADA Home Health Care is seeking an experienced Weekend Physical Therapist to provide care to adult and geriatric patients in their homes in Hyannis, Yarmouth, Dennis, and surrounding towns. The Physical Therapist is an integral member of a multidisciplinary health care team, providing skilled rehabilitative care that helps clients maintain their independence. The role offers a flexible per diem or part-time schedule and premium weekend pay.

The Physical Therapist visits clients in their homes, evaluates mobility and other functional abilities, develops and revises individualized care plans, and provides therapeutic interventions. The role also includes educating clients and their representatives about rehabilitative care and documenting care using a touch pad tablet.

Requirements

• Current Massachusetts license as a Physical Therapist.
• At least one year of recent clinical experience as a licensed Physical Therapist.

Nice To Haves

• Home health experience is strongly preferred.

Responsibilities

• Make visits to clients in their homes.
• Perform diagnostic tests and measurements, including assessments of joint mobility and range, transfer status, stability, gait patterns and appearance, muscle strength and endurance, balance, and safety.
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance work, and other interventions.
• Continually assess and revise the Physical Therapy care plan and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives in rehabilitative care and activities that promote health, safety, and independent living.
• Accurately document observations, interventions, and evaluations related to client care management and services provided using a touch pad tablet.
