Weekend Home Health Physical Therapist
About The Position
BAYADA Home Health Care is seeking an experienced Weekend Physical Therapist to provide care to adult and geriatric patients in their homes in Hyannis, Yarmouth, Dennis, and surrounding towns. The role is part of a multidisciplinary health care team providing skilled rehabilitative care that helps clients maintain their independence. The schedule is flexible and per diem or part-time, with premium pay and a weekend differential. Prior home health experience is strongly preferred. The position pays $65.00–$72.00 per point, depending on qualifications, plus a weekend differential. Requirements
- Current license as a Physical Therapist in Massachusetts.
- At least one year of recent clinical experience as a licensed Physical Therapist.
Nice To Haves
- Home health experience is strongly preferred.
Responsibilities
- Make visits to clients in their homes.
- Perform diagnostic tests and measurements, including assessing joint mobility and range of motion, transfer status, stability, gait patterns and appearance, muscle strength and endurance, balance, and safety.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance activities, and other interventions.
- Continually assess and revise the Physical Therapy care plan and participate with nursing in the multidisciplinary care plan, as appropriate.
- Educate and instruct clients, family members, or other client representatives in rehabilitative care and activities that promote health, safety, and independent living.
- Accurately document observations, interventions, and evaluations related to client care management and services using a touch pad tablet.
