Physical Therapist (PT) - Skilled Nursing Facility (SNF) - Part-Time
About The Position
Concept Rehab is seeking a Part-Time Physical Therapist (PT) to join their team in a Skilled Nursing Facility (SNF) in Delaware, Ohio. This role involves working with adult and geriatric patients who are receiving post-acute rehabilitation, skilled nursing care, and long-term care services. The Physical Therapist will evaluate and treat patients recovering from various conditions such as illness, injury, surgery, hospitalization, and functional decline. A key aspect of the role is collaborating with physicians, nursing staff, other rehabilitation professionals, patients, and their families to create individualized treatment plans aimed at enhancing mobility, strength, balance, safety, overall function, and quality of life. The company is looking for individuals passionate about patient-centered care and dedicated to helping patients achieve their maximum independence.
Requirements
- Bachelor's, Master's, or Doctorate Degree in Physical Therapy (BS, MS, MPT, or DPT)
- Current Ohio Physical Therapist license in good standing
- Strong clinical reasoning and patient care skills
- Excellent communication and documentation abilities
- Ability to work collaboratively within an interdisciplinary rehabilitation team

Responsibilities
- Perform physical therapy evaluations within established timelines following physician referral.
- Develop individualized plans of care based on patient needs and goals.
- Provide evidence-based physical therapy treatment interventions.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Complete discharge planning and recommendations.
- Communicate effectively with physicians, nurses, rehabilitation staff, patients, and family members.
- Participate in interdisciplinary team meetings and care planning discussions.
- Educate patients and caregivers regarding treatment goals, safety, and home or maintenance programs.
- Recommend adaptive equipment and therapeutic interventions that support optimal outcomes.
- Maintain accurate, timely, and professional clinical documentation.
- Complete treatment notes, progress reports, discharge summaries, and billing documentation in accordance with company policies and regulatory standards.
- Maintain productivity expectations while delivering high-quality patient care.
- Supervise Physical Therapist Assistants (PTAs) as required by state regulations.
- Provide in-service education and training to facility staff.
- Support restorative nursing and functional maintenance programs.
- Serve as a clinical instructor for affiliated physical therapy educational programs when applicable.
