About The Position

We are seeking a compassionate and skilled Physical Therapist (PT) for a 13-week contract position in a Skilled Nursing Facility (SNF) in Marysville, California. The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide rehabilitative care focused on improving mobility, restoring function, and maximizing independence. The PT will work closely with an interdisciplinary care team while providing services to patients receiving long-term care and short-term rehabilitation. The Physical Therapist will support residents in improving functional mobility, safety, and independence through individualized rehabilitative care.

Requirements

• Graduate of an Accredited Physical Therapy Program.
• Current Active California Physical Therapist License Required.
• Strong Communication, Documentation, and Interpersonal Skills.
• Ability to Work Independently and Collaboratively as Part of a Multidisciplinary Team.

Nice To Haves

• Skilled Nursing Facility or Geriatric Rehabilitation Experience Preferred.

Responsibilities

• Evaluate patients' physical abilities, mobility limitations, functional status, and pain levels.
• Develop and implement individualized treatment plans based on clinical assessments, medical history, and patient goals.
• Provide evidence-based physical therapy interventions to improve strength, balance, coordination, mobility, and functional independence.
• Monitor patient progress, adjust treatment plans as appropriate, and maintain timely clinical documentation.
• Collaborate with the interdisciplinary care team and supervise Physical Therapist Assistants (PTAs) and aides as needed.
