About The Position

We are seeking an experienced Physical Therapist (PT) for a 13-week contract position in a Skilled Nursing Facility (SNF) in Lawrence, Massachusetts. The Physical Therapist will evaluate residents, develop individualized plans of care, and provide therapeutic interventions designed to improve functional mobility, independence, and activities of daily living. This position requires previous skilled nursing experience and involves close collaboration with residents, caregivers, physicians, Physical Therapist Assistants, and interdisciplinary healthcare professionals. The Physical Therapist will provide resident-centered rehabilitation services focused on maximizing functional ability, safety, and independence.

Requirements

• Active Massachusetts Physical Therapist (PT) License Required.
• Bachelor's Degree in Physical Therapy or Related Qualifying Physical Therapy Education Required; Master's or Doctorate Preferred.
• Minimum 3 Years of Physical Therapy Experience Required With Previous Skilled Nursing Facility Experience.
• Current BLS/BCLS Certification Required.
• Strong Clinical Assessment, Documentation, Communication, Time Management, and Interdisciplinary Collaboration Skills Required.

Responsibilities

• Evaluate residents and develop individualized treatment plans with measurable short- and long-term functional goals.
• Provide therapeutic interventions and monitor patient response, modifying treatment plans as clinically appropriate.
• Educate residents and caregivers regarding therapy goals, assistive devices, mobility techniques, and recommended care strategies.
• Collaborate with physicians, caregivers, Physical Therapist Assistants, and interdisciplinary team members regarding treatment and discharge planning.
• Maintain accurate clinical documentation in accordance with Medicare, Medicaid, reimbursement, regulatory, and facility requirements.
