About The Position
Innovate Rehab and Wellness is seeking a PRN Physical Therapist to work in a skilled nursing facility. The therapist evaluates patients, identifies movement and functional limitations, and creates and leads individualized treatment plans to help restore movement, function, and quality of life. The role works closely with an interdisciplinary team to support optimal recovery. The position offers a flexible, as-needed schedule and emphasizes one-on-one patient care, with group or concurrent treatments accounting for less than 1% of care. Requirements
- Doctorate or master’s degree in Physical Therapy from an accredited program.
- Valid state physical therapist license.
- Strong communication, problem-solving, and patient-care skills.
Nice To Haves
- Clinical experience preferred; new graduates are welcome.
- CPR certification preferred.
Responsibilities
- Perform comprehensive physical therapy evaluations.
- Develop personalized, evidence-based treatment plans.
- Provide hands-on therapy, including manual techniques, therapeutic exercise, and mobility training.
- Educate patients and caregivers about home programs and injury prevention.
- Track and document patient progress and adjust treatment plans as needed.
- Collaborate with physicians, nurses, occupational therapists, and other clinicians.
- Stay current with physical therapy best practices through continuing education.
- Ensure compliance with regulatory and ethical standards.
