Physical Therapist (SIGN ON BONUS) - Hackensack, NJ
About The Position
FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in Hackensack, Garfield, Lodi, and surrounding areas. Care is delivered in patients’ private residences and senior living communities, including independent living, assisted living, and memory care settings. This position is partially staffed at the Felician Sisters Convent in Lodi. Clinicians use FOX’s Geriatric House Calls™ model to help patients improve mobility, independence, wellness, and quality of life. Telehealth and other virtual care may also be incorporated when appropriate. The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and collaborate with an interdisciplinary team. FOX lists full-time, part-time, and PRN/Fox Flex employment pathways. New graduates may be eligible for the Emerging Professionals Mentor Program, and H1B sponsorship is available for qualified clinicians. The standard New Jersey base pay range for a full-time role is $70,000–$102,000 annually, with an opportunity to earn more. Part-time and PRN compensation is prorated. Compensation depends on experience and employment type.
Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, CPR, patient transfers and bed mobility, repositioning patients, extended standing and walking, stair climbing, and therapeutic activities.
- Ability to drive to patient homes and senior living communities.
- Ability to wear PPE when required.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living communities.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Deliver therapy sessions that support wellness, longevity, mobility, and functional independence.
- Document in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
- When applicable, supervise PTAs/OTAs and support students or emerging professionals through mentoring or clinical instruction.
