Physical Therapist (SIGN ON BONUS) - Lakewood, NJ
About The Position
FOX Rehabilitation is hiring licensed Physical Therapists in Lakewood, New Jersey, to provide one-on-one care to older adults in private residences and senior living communities, including The Fairways and Lake Ridge. Using FOX’s Geriatric House Calls™ model, clinicians help patients improve mobility, independence, wellness, and quality of life. Care may also include telehealth and other virtual services when appropriate. This role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and work in a collaborative, interdisciplinary community-based practice. FOX offers full-time, part-time, and PRN/FOX Flex employment options. New graduates are eligible for the Emerging Professionals Mentor Program, and H1B sponsorship is available for qualified clinicians. Compensation varies by experience and employment type. In New Jersey, the standard annual base-pay range for a full-time role is $70,000–$102,000, with an opportunity to earn more; part-time and FOX Flex compensation is prorated.
Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
- Ability to wear PPE when required.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
- Document effectively in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
