Physical Therapist - Full Time
About The Position
Preferred Care Home Health is hiring a full-time Physical Therapist to provide home health services in Naples, Florida, serving Collier County. The therapist will help patients improve mobility, regain independence, and achieve their goals at home through evaluations, individualized treatment plans, skilled therapy, patient and caregiver education, and coordination with physicians and the interdisciplinary care team. The position includes supervising Physical Therapist Assistants, participating in case conferences and care planning, and engaging in ongoing clinical education. The schedule is Monday through Friday, 8:30 a.m. to 5:00 p.m.
Requirements
- Degree from an accredited Physical Therapy program.
- Active, unrestricted Physical Therapist license in good standing.
- Valid driver’s license, reliable transportation, and auto insurance.
- Strong clinical judgment, communication, and documentation skills.
Nice To Haves
- Home health experience is preferred but not required.
Responsibilities
- Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
- Provide skilled therapy services in accordance with physician orders and established care plans.
- Monitor patient progress and adjust interventions to support optimal outcomes.
- Educate patients and caregivers on therapeutic exercises, safety techniques, and the use of assistive equipment.
- Complete accurate and timely clinical documentation.
- Communicate changes in patient condition to physicians and the interdisciplinary care team.
- Provide clinical supervision to Physical Therapist Assistants (PTAs).
- Participate in case conferences, care planning, and ongoing clinical education.
