About The Position
Concept Rehab is seeking a licensed Physical Therapist (PT) to provide high-quality rehabilitation services in a post-acute rehabilitation setting. This is a flexible PRN opportunity where you can build your own schedule while making a meaningful impact. The role involves helping patients improve mobility, restore function, and achieve greater independence. As a PRN Physical Therapist, you'll have the flexibility to pick up shifts that fit your schedule while working with an experienced interdisciplinary team dedicated to patient-centered care.
Requirements
- Doctor of Physical Therapy (DPT) or equivalent from an accredited program.
- Current Physical Therapist license in the State of Michigan.
- Strong clinical decision-making and communication skills.
Nice To Haves
- Experience in rehabilitation, post-acute care, or geriatric therapy is welcomed but not required.
- New graduates are encouraged to apply.
Responsibilities
- Perform comprehensive physical therapy evaluations.
- Develop individualized treatment plans based on each patient's goals and functional abilities.
- Deliver evidence-based physical therapy interventions to improve strength, balance, mobility, endurance, and safety.
- Monitor patient progress and adjust treatment plans as appropriate.
- Collaborate with Occupational Therapy, Speech Therapy, nursing, physicians, and caregivers.
- Complete accurate, timely clinical documentation.
