About The Position

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in Berkeley, California, and surrounding communities. Care is delivered in private residences and senior living settings, including independent living, assisted living, and memory care, through FOX’s Geriatric House Calls™ model. Clinicians evaluate patients, develop individualized plans of care, and provide evidence-based treatment that supports wellness, mobility, functional independence, and quality of life. Telehealth and other virtual care may also be incorporated when appropriate. The role offers autonomy to create and manage a treatment schedule and develop lasting patient relationships within a collaborative, interdisciplinary practice. FOX offers full-time, part-time, and PRN/FOX Flex employment pathways. Full-time roles are salaried with benefits; part-time benefits eligibility is based on minimum hours, and PRN/FOX Flex compensation is per unit. H1B sponsorship is available for qualified clinicians, and new graduates may participate in the Emerging Professionals Mentor Program. The standard base pay range for a full-time role in this California region is $80,000–$114,000 annually, with an opportunity to earn more. Part-time and FOX Flex compensation is prorated. Requirements

Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
Degree from an accredited Physical Therapy program.
Basic computer and EMR proficiency.
Current CPR certification.
Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
Ability to wear PPE when required.

Responsibilities

Provide one-on-one physical therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
Document in FOX’s EMR system in accordance with regulatory standards.
Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Provide telehealth or virtual care when appropriate.
Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
