Home Care Physical Therapist- Per Diem (Union County)
About The Position
Ignite Rehab, operating as part of Metro Physical & Aquatic Therapy, is a multidisciplinary therapy provider offering Physical and Occupational Therapy services. Metro is seeking a motivated, passionate Physical Therapist to provide in-home care in the Union County area. Candidates at all experience levels are encouraged to apply, and training is available for new graduates. The role offers autonomy and flexibility, including the opportunity to see patients any seven days of the week, with preference given to clinicians serving their primary geographic area.
Requirements
- Graduate of a Physical Therapist program.
- Licensed Physical Therapist in New Jersey or eligible for licensure.
- Ability to travel to patients’ homes within the assigned geographic territory.
Nice To Haves
- Home health experience is preferred but not required.
Responsibilities
- Accept and treat patients within the designated geographic area.
- Evaluate patients’ physical therapy needs in the home setting and develop individualized treatment plans.
- Develop plans of care and treatment goals in consultation with the physician, patient, and family.
- Plan, prepare, and implement customized therapy programs to restore function and promote independence.
- Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
- Provide timely and accurate documentation for each visit.
- Communicate effectively with patients, families, and interdisciplinary team members, particularly regarding discharge planning.
- Deliver care in compliance with agency policies and professional practice standards.
