Physical Therapist - Temporary. Fixed Term Position

About The Position
PACE North is seeking a licensed Physical Therapist to join their team on a temporary, fixed-term basis to provide coverage during a maternity leave. This role involves leading and delivering high-quality care to the aging population, both at the Center and in participants' homes, with the goal of helping older adults maintain their independence and improve their quality of life. The position is full-time with negotiable hours, Monday through Friday, and offers a great work-life balance with no weekends or holidays.
Requirements
- Bachelor’s in physical therapy, Master’s, or Doctorate
- Licensed Physical Therapist in Michigan
- CPR Certification (We can train)
- Valid Michigan Driver’s License
- Strong clinical competency in geriatric care
- Basic computer proficiency
- Ability to work independently and collaboratively
- Clear communication and documentation skills
- Commitment to participant-centered care and quality improvement
Responsibilities
- Conduct initial and periodic assessments of participants’ physical and functional status
- Develop personalized care and treatment plans, including therapeutic exercise, gait training, and mobility support
- Collaborate with an interdisciplinary care team and communicate changes in participant condition
- Recommend and evaluate the need for durable medical equipment
- Train family members, caregivers, and staff on implementing therapy strategies
- Contribute to participant discharge planning and continuity of care across settings
