About The Position

Integrity Home Care + Hospice is recruiting a compassionate, part-time Home Health Physical Therapist to serve patients in Springfield, Missouri, and the surrounding area. The therapist provides one-to-one clinical care in clients’ homes, helping them recover from injury, illness, surgery, or hospitalization. The role includes home visits in designated geographic territories and participation in multidisciplinary care planning. Training is provided, with hands-on instruction supplemented by virtual learning and 24/7 clinical support from an interdisciplinary team.

Requirements

• Active and current physical therapist license.
• Ability to travel locally to assigned cases.

Nice To Haves

• One year of clinical experience is preferred.
• Previous home health care experience is welcomed but not required; training is provided.

Responsibilities

• Make home visits to clients in assigned geographic territories.
• Perform diagnostic tests and measurements, including assessments of joint mobility and range of motion, transfers, stability, gait, muscle strength and endurance, balance, and safety.
• Implement evidence-based integumentary protective interventions to prevent wounds and promote wound repair, using dressings and topical agents.
• Develop and implement individualized care plans, including therapeutic exercises, gait training, balance activities, and other interventions.
• Continuously assess and revise physical therapy care plans and participate in multidisciplinary care planning with nursing as appropriate.
• Educate and instruct clients, family members, and other representatives in rehabilitative care and activities that promote health, safety, and independent living.
• Accurately document observations, interventions, evaluations, and services provided using a touch-pad tablet.
