The physical therapist contracted or employed through the Organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement. This role involves the ongoing interdisciplinary assessment and development of individualized plans of care in partnership with patients, caregivers, and families. The therapist will provide patient and caregiver education, prepare clinical notes, and communicate with physicians and other healthcare practitioners regarding the home health plan of care. Services include assessing and evaluating therapeutic/rehabilitative/functional status, directing physical therapy treatment, instructing patients and families on the use of therapeutic appliances, and identifying needs for other home health services. The role also includes training patients in the use of prosthetic devices, participating in discharge planning, providing in-service education, and participating in quality assessment and performance improvement programs. When therapy is the only skilled service, the therapist will instruct, supervise, and evaluate home health aide care every two weeks and supervise Physical Therapy Assistants according to organizational policy and state regulations.
