About The Position

FOX Rehabilitation is hiring a Physical Therapist in the Greater Stoughton, Wisconsin area, with an Emerging Professionals Mentor Program designed for newly graduated professionals. The program provides a dedicated mentor and hands-on training to support professional development and help clinicians become confident, well-rounded practitioners. Prior in-home or home health experience is not required. FOX provides outpatient services in patients’ homes, emphasizing clinician flexibility and autonomy and one-to-one patient care. Physical Therapists see an average of 6–8 patients per day and provide individualized 60-minute sessions without double-booking. Clinicians set their own schedules; nights and weekends are not required. Additional income may be available by treating additional patients, and referral bonuses are offered for referring a classmate or colleague. Requirements

Be licensed or soon to be licensed as a Physical Therapist in Wisconsin.
Graduate from an accredited physical therapy program.
Hold current CPR certification.
Demonstrate exceptional patient service, strong interpersonal skills, and consistent clinical excellence.
Show a desire to develop professionally and a passion for patient-centered care.
Have basic computer literacy.

Responsibilities

Provide outpatient physical therapy services to patients in their homes.
Deliver individualized, one-to-one care in 60-minute sessions without double-booking.
Manage an average caseload of 6–8 patients per day.
Set and manage a flexible work schedule.
Develop professionally through the Emerging Professionals Mentor Program and hands-on training.
