Seeking a full-time Physical Therapist to join the rehabilitation team in Warren, Michigan. The role focuses on helping older adult residents improve mobility, strength, balance, coordination, and overall function, and supporting their functional independence and quality of life.
Requirements
- Current Michigan Physical Therapist license.
- Graduate of an accredited Physical Therapy program.
- Strong clinical assessment and treatment skills.
- Dependable, team-oriented professional with a passion for patient care.
Nice To Haves
- Previous skilled nursing facility (SNF) or geriatric rehabilitation experience.
Responsibilities
- Evaluate residents and develop individualized physical therapy treatment plans.
- Provide therapeutic interventions to improve mobility, strength, balance, and coordination.
- Monitor patient progress and adjust treatment plans as needed.
- Document evaluations, treatments, and progress according to clinical guidelines.
- Communicate with residents, families, and interdisciplinary care teams.
- Help residents work toward functional independence and improved quality of life.
