Requirements

  • Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
  • Master’s degree in Physical Therapy; OR
  • Master’s degree in a Physical Therapy related field in addition to a Bachelor’s degree in Physical Therapy; OR
  • Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience.
  • The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy.
  • Valid driver’s license or reliable, independent transportation to and from the patients’ residence.

Responsibilities

  • Provide outpatient rehabilitation services in an in-home setting for the older adult population.
  • Evaluate and treat patients in their home environment, which could be a private residence or a patient's room in an independent living facility.
  • Assess the need for, develop, and deliver physical therapy programs to facilitate rehabilitation.
  • Identify the need for and make referrals to other disciplines to address the comprehensive needs of patients.
  • Implement individualized therapy programs designed to restore, reinforce, and enhance performance.
  • Instruct, educate, and train patients and caregivers in skills and functions essential for promoting independence and productivity, in order to diminish or correct pathology.
  • Direct activities to promote and maintain health.
  • Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel as necessary or applicable, in accordance with state licensure requirements and professional standards.
  • Adhere to department and patient treatment schedules, timely clinical and billing documentation requirements, and work-related meetings.
  • Attend and contribute to patient care, staffing conferences, and other related meetings.
  • Consult with other members of the rehabilitation team to select the most appropriate therapeutic regimen consistent with the needs and capabilities of each patient.
  • Adapt program and/or plan of care according to the needs of the individual patient.
  • Promote a safe environment and enforce the elimination of fire and safety hazards.
  • Order supplies and equipment as necessary.
  • Clean, maintain, identify, and report the need for repair of equipment as necessary.
  • Prioritize patient service by ensuring patients and families receive the highest quality of service in a caring and compassionate atmosphere that recognizes individual needs and rights.
  • Screen, examine, and evaluate patients, including history, systems review, and application of appropriate tests and measures.
  • Synthesize information and interpret examination findings to establish a diagnosis, identify impairments, determine the predicted level of improvement and the time required to achieve it, identify precautions/contraindications, and design and plan of care.
  • Develop appropriate treatment goals and methods in collaboration with the patient and caregivers.
  • Implement the physical therapy treatment plan and complete all related documentation and record keeping.
  • Provide therapeutic exercise including aerobic capacity conditioning, balance coordination, agility training, body mechanics and postural stabilization, flexibility exercises, gait and locomotion training, neuromotor development training, relaxation, strength, power, and endurance training.
  • Provide functional training.
  • Provide manual therapy.
  • Provide airway clearance techniques.
  • Provide integumentary repair and protection.
  • Utilize electrotherapeutic, physical agent, and mechanical modalities.
  • Incorporate health and wellness principles.
  • Provide education on chronic disease management.
  • Perform falls assessment and interventions to reduce fall risk.
  • Provide case management in the home and community.
  • Design, fabricate, fit, and/or apply adaptive devices, and train patients in the use of adaptive devices including orthotics and prosthetics.
  • Select appropriate activities suited to the individual’s physical and mental capacity, intelligence level, and interest.
  • Instruct patients in the skills and techniques required for participation in mobility activities.
  • Evaluate patient progress, attitudes, and behavior as related to rehabilitative potential.
  • Perform routine reexamination as needed/required to modify/progress plan of treatment.
  • Incorporate health and wellness programming into treatment plans for patients.
  • Perform home and community mobility assessments and provide recommendations for safe mobility in the home and community space.
  • Perform other related duties as required.
