Requirements

  • Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
  • Master’s degree in Physical Therapy; OR
  • Master’s degree in a Physical Therapy related field in addition to a Bachelor’s degree in Physical Therapy; OR
  • Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience.
  • Qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent.
  • Valid driver’s license or reliable, independent transportation to and from the patients’ residence.

Responsibilities

  • Provide outpatient rehabilitation services in an in-home setting for the older adult population.
  • Evaluate and treat patients in their home environment, which could be a private residence or a room in an independent living facility.
  • Assess the need for, develop, and deliver physical therapy programs to facilitate rehabilitation.
  • Identify the need for and make referrals to other disciplines to address the comprehensive needs of patients.
  • Implement individualized therapy programs designed to restore, reinforce, and enhance performance.
  • Instruct, educate, and train patients and caregivers in skills and functions essential for promoting independence and productivity.
  • Direct activities to promote and maintain health.
  • Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel as applicable.
  • Adhere to department and patient treatment schedules, timely clinical and billing documentation requirements, and work-related meetings.
  • Attend and contribute to patient care and staffing conferences.
  • Consult with other members of the rehabilitation team to select the most appropriate therapeutic regimen.
  • Adapt program and/or plan of care according to the needs of the individual patient.
  • Promote a safe environment and enforce the elimination of fire and safety hazards.
  • Order supplies and equipment as necessary.
  • Clean, maintain, identify, and report the need for repair of equipment.
  • Put patient service first: ensure patients and families receive the highest quality of service in a caring and compassionate atmosphere.
  • Screen, examine, and evaluate patients, including history, systems review, and application of appropriate tests and measures.
  • Synthesize information and interpret examination findings to establish a diagnosis, identify impairments, determine the predicted level of improvement and time required, identify precautions/contraindications, and design a plan of care.
  • Develop appropriate treatment goals and methods in collaboration with the patient and caregivers.
  • Implement the physical therapy treatment plan and complete all related documentation and record keeping.
  • Provide services including therapeutic exercise, functional training, manual therapy, airway clearance techniques, integumentary repair and protection, use of electrotherapeutic, physical agent and mechanical modalities, health and wellness, education on chronic disease management, falls assessment and interventions, and case management.
  • Design, fabricate, fit, and/or apply adaptive devices, and train patients in their use.
  • Select appropriate activities suited to the individual’s physical and mental capacity, intelligence level, and interest.
  • Instruct patients in skills and techniques required for participation in mobility activities.
  • Evaluate patient progress, attitudes, and behavior as related to rehabilitative potential.
  • Perform routine reexamination as needed/required to modify/progress plan of treatment.
  • Incorporate health and wellness programming into treatment plans.
  • Perform home and community mobility assessments and provide recommendations for safe mobility.
  • Perform other related duties as required.
