Physical Therapist (RELOCATION BONUS) - Peekskill, NY
About The Position
FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in Peekskill, NY, and surrounding locations. Care is delivered in private residences and senior living communities through the Geriatric House Calls™ model, with the goal of improving mobility, independence, wellness, and quality of life. Clinicians may also provide telehealth and other virtual care when appropriate. The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and work within a clinician-driven, interdisciplinary practice. FOX offers full-time, part-time, and PRN/Fox Flex pathways; compensation and benefit eligibility vary by employment type. New graduates may be eligible for the Emerging Professionals Mentor Program.
Requirements
- Valid Physical Therapy license in the state or states of practice, or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
- Ability to wear PPE when required.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
