About The Position

Human Touch Home Health is seeking a New Graduate Physical Therapist (PT) for PRN home health opportunities throughout the Fredericksburg area. This role offers the chance to build clinical confidence, gain hands-on experience, and make an impact one patient at a time. Patient opportunities are available in Fredericksburg, Stafford, Spotsylvania, King George, Caroline County, Orange, Locust Grove, Bowling Green, Colonial Beach, and surrounding communities. You can build your PRN caseload around your availability and preferred service areas.

Requirements

• Active Virginia Physical Therapist License Required
• Graduate of an accredited Physical Therapy program
• Valid Driver's License and reliable transportation
• Strong communication and patient-care skills
• Willingness to learn and grow in the home health setting
• Ability to work independently while utilizing available clinical support

Nice To Haves

• No previous home health experience is required.

Responsibilities

• Complete physical therapy evaluations in patients' homes
• Develop individualized plans of care
• Provide therapeutic exercise, gait, balance, transfer, and mobility training
• Assess home safety and fall risks
• Educate patients and caregivers
• Monitor patient progress and adjust treatment plans
• Coordinate care with physicians, nurses, therapists, and other team members
• Complete accurate and timely documentation using WellSky
