About The Position

Human Touch Home Health is seeking a New Graduate Physical Therapist (PT) to join our Blacksburg team serving patients throughout Blacksburg, Christiansburg, Radford, and surrounding New River Valley communities. This role offers an opportunity to build confidence, strengthen clinical skills, and grow into the therapist you trained to become. Experience isn't required to bring value, and your career has to start somewhere. As a new PT, home health gives you a unique opportunity to understand your patients beyond the treatment table. You'll see their actual stairs, bathrooms, mobility challenges, and daily routines—and learn how to build treatment plans around real life. We're looking for a licensed new graduate who's ready to learn, willing to ask questions, and excited to become an excellent Physical Therapist.

Requirements

• Graduate of an Accredited Physical Therapy Program
• Active Virginia Physical Therapist License Required
• Valid Driver's License and reliable transportation
• Strong communication and patient-care skills
• Willingness to learn and grow in the home health setting
• Ability to work independently while knowing when to ask for support

Nice To Haves

• No previous home health experience required.

Responsibilities

• Complete physical therapy evaluations in patients' homes
• Develop individualized plans of care
• Provide therapeutic exercise, gait, balance, transfer, and mobility training
• Assess home safety and fall risks
• Educate patients and caregivers
• Monitor patient progress
• Coordinate care with nurses, therapists, physicians, and other team members
• Complete timely documentation using WellSky
• Participate in discharge planning
