About The Position
The Physical Therapist is responsible and accountable for residents’ evaluations, ongoing assessments, and the planning and implementation of therapy interventions. This role involves developing and revising plans of care based on resident needs, establishing functional goals, outcomes, and discharge plans. The therapist will also plan for resident discharge, anticipate long-term and/or short-term resident needs, and educate other healthcare team members, residents, family, and caregivers on information regarding the care plan.
Requirements
- Current state licensure in Physical Therapy, in good standing.
- Bachelor’s degree.
