About The Position
FOX Rehabilitation is seeking passionate, licensed Physical Therapists in Burlington, New Jersey, to provide exceptional one-on-one care to older adults in their homes or within Senior Living Communities. The role focuses on improving patient mobility, independence, and quality of life through the Geriatric House Calls™ model. This position offers autonomy, flexibility, and the opportunity to manage one's own treatment schedule and build strong patient relationships. Clinicians may also incorporate telehealth and virtual care, enhancing flexibility and patient support. This is a high-impact PT role aimed at positively impacting the lives of the communities served.
Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification

Responsibilities
- Provide one-on-one physical therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
- Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
- Document effectively within FOX’s EMR system in accordance with regulatory standards
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
- Provide telehealth / virtual care when appropriate to extend access and flexibility
