About The Position
FOX Rehabilitation is seeking passionate, licensed Physical Therapists in Hackensack, New Jersey, to provide exceptional one-on-one care to older adults in their private residences or Senior Living Communities. This role focuses on improving patients' mobility, independence, and quality of life through the signature Geriatric House Calls™ model. The position offers autonomy and flexibility, allowing therapists to create and manage their own treatment schedules and build strong, lasting patient relationships. Clinicians may also incorporate telehealth and virtual care. FOX Rehabilitation is a clinician-operated practice dedicated to meaningful relationships, evidence-based care, and abolishing ageism, providing support from leaders who understand clinical work. Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
Responsibilities
- Provide one-on-one physical therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
- Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
- Document effectively within FOX’s EMR system in accordance with regulatory standards
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
- Provide telehealth / virtual care when appropriate to extend access and flexibility
