About The Position

Active Fitness & Physical Therapy, an H2 Health company, is hiring a Physical Therapist for its outpatient clinics. The therapist will provide personalized care for a diverse range of diagnoses, including pre- and post-operative conditions, spinal disorders, gait and balance deficits, orthopedic and neurological conditions, and sports-related conditions. Clinicians may maintain a general caseload or specialize in areas such as orthopedics, manual therapy, hand therapy, prosthetic rehabilitation, vestibular care, or concussion care. The role involves developing individualized plans of care, collaborating with healthcare providers, and documenting treatment in Raintree EMR. The therapist will maintain a caseload of approximately 50 visits per week, with built-in time for documentation, and participate in ongoing professional development and team initiatives. The position is full-time at the outpatient clinic at 44 Opossum Creek Rd, Victor, WV 25938.

Requirements

Degree from an accredited Physical Therapy program.
Physical Therapy license or eligibility for licensure in the state of practice.
Strong communication and clinical reasoning skills.

Responsibilities

Assess and treat patients with orthopedic, neurological, pre- and post-operative, and sports-related conditions.
Develop individualized outpatient physical therapy plans of care to optimize patient outcomes.
