About The Position

Preferred Care Home Health is hiring a full-time Physical Therapist to provide home health services in Vero Beach and surrounding areas of Indian River County. The role focuses on helping patients improve mobility, regain independence, and achieve their goals at home. The position is scheduled Monday through Friday, 8:30 a.m. to 5:00 p.m.

Requirements

• Degree from an accredited Physical Therapy program.
• Active, unrestricted Physical Therapist license in good standing.
• Valid driver’s license, reliable transportation, and auto insurance.
• Strong clinical judgment, communication, and documentation skills.

Nice To Haves

• Home health experience is preferred but not required.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
• Provide skilled therapy services in accordance with physician orders and established care plans.
• Monitor patient progress and adjust interventions to support optimal outcomes.
• Educate patients and caregivers on therapeutic exercises, safety techniques, and the use of assistive equipment.
• Complete accurate and timely clinical documentation.
• Communicate changes in patient condition to physicians and the interdisciplinary care team.
• Provide clinical supervision to Physical Therapist Assistants (PTAs).
• Participate in case conferences, care planning, and ongoing clinical education.
