About The Position

Concordia Visiting Nurses is seeking a skilled and compassionate Physical Therapist to provide home health therapy services in the Brentwood to West Mifflin area. Requirements

Graduate of an accredited Physical Therapy program.
Valid state license.
Passion for working with patients and commitment to high-quality, patient-centered care.
Exceptional communication and interpersonal skills, including the ability to connect with diverse patient populations.

Desire to make a difference in patients’ lives and inspire and motivate them during recovery.

Responsibilities

Conduct comprehensive evaluations and develop individualized treatment plans to help patients reach their maximum potential.
Implement evidence-based therapeutic interventions, including manual therapy, therapeutic exercises, and modalities.
Monitor patient progress, adjust treatment plans as needed, and complete timely, thorough documentation.
Educate patients and their families about treatment options, rehabilitation techniques, and preventative measures.
Collaborate with physicians and other healthcare professionals to support holistic patient care.
Help create a welcoming and supportive environment where patients feel comfortable and motivated.
