About The Position

Post Rehabilitation Services is seeking a full-time Physical Therapist at Civita Care Center West River in Milford, Connecticut. The therapist helps skilled nursing residents rebuild physical abilities, address mobility barriers, and prepare for their next stage of care. The role includes assessing residents, developing individualized plans of care, delivering therapy, tracking progress, documenting treatment, and collaborating with nursing and rehabilitation colleagues on discharge recommendations. The position offers $45–$50 per hour and benefits.

Requirements

• Active Connecticut Physical Therapist license.
• Strong clinical judgment, assessment, and treatment planning skills.
• Effective communication skills.
• Timely and accurate documentation.

Responsibilities

• Assess residents’ movement, strength, balance, and functional abilities to identify treatment priorities.
• Design individualized plans of care that account for medical precautions, previous function, and resident goals.
• Provide therapeutic exercise, gait training, and task-specific practice for transfers and daily mobility.
• Evaluate assistive device needs and train residents and caregivers in safe movement strategies.
• Measure progress, update treatment plans, and provide clinical direction to physical therapist assistants.
• Complete evaluations and treatment records.
• Partner with nursing and rehabilitation colleagues on discharge recommendations.
