About The Position

Human Touch Home Health is seeking compassionate Physical Therapists (PTs) for PRN home health opportunities throughout Fredericksburg, Stafford, Spotsylvania, and nearby communities. This role offers a flexible schedule where you can choose your territory and build a caseload that works for you. You will be responsible for completing evaluations, developing individualized plans of care, providing various therapeutic exercises and training, assessing home safety, educating patients and caregivers, monitoring progress, and coordinating care with an interdisciplinary team. Documentation will be completed using WellSky.

Requirements

• Active Virginia Physical Therapist License Required
• Graduate of an accredited Physical Therapy program
• Valid Driver's License and reliable transportation
• Strong clinical and communication skills
• Ability to work independently in patients' homes

Nice To Haves

• Home Health Experience Preferred, but Not Required
• WellSky Experience is a Plus

Responsibilities

• Complete physical therapy evaluations in patients' homes
• Develop individualized plans of care
• Provide therapeutic exercise, gait, balance, transfer, and mobility training
• Help patients improve strength, endurance, and functional independence
• Assess home safety and identify fall risks
• Educate patients and caregivers on mobility and home exercise programs
• Monitor patient progress and adjust treatment plans as needed
• Complete reassessments and discharge planning
• Coordinate care with the interdisciplinary team
• Complete accurate and timely documentation using WellSky
