Requirements

  • Bachelor of Science in Physical Therapy
  • Current Texas State License as a Physical Therapist
  • 1 year of minimum experience as a Physical Therapist in Home Healthcare, Acute care, or Rehabilitation setting
  • Therapy skills as defined as generally accepted standards of practice.
  • Good communication and interpersonal skills.
  • Proof of current CPR and Hepatitis profile.

Nice To Haves

  • Spanish is a Plus
  • Knowledge of durable medical equipment

Responsibilities

  • Provide coordinated care to patients and evaluate patient and restore or maintain patient well-being according to the plan of care.
  • Evaluate and provide a treatment plan for a patient with physical, psycho-social, and/or cognitive impairments.
  • Provide intervention to the deficit and elicit responses.
  • Collaborates with interdisciplinary team.
  • Perform initial and ongoing clinical assessments to identify patient needs, determine the level of function at appropriate time points, and the appropriateness of the patient for Home health.
  • Develop and revise a plan of care in consultation with the physician and other care team members.
  • Provide therapeutic treatments, evaluate need for and use of adaptive equipment and environmental modifications, fabricate and maintain adaptive equipment and hand splints as applicable.
  • Evaluate outcomes of care with appropriate discharge planning.
  • Apply concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
  • Accept clinical assessments that are consistent with education and competence to care for patients.
  • Supervise PTA as needed.
  • Complete appropriate documentation in a timely manner.
  • Meet the mandatory continuing education requirements of the agency/license board.
  • Use effective interpersonal relations and communication skills to advise and consult with the family and other agency personnel as appropriate.
  • Demonstrate commitment, professional growth, and competency.
  • Promote Agency philosophy and administrative policies to ensure quality of care.
