About The Position
We are seeking a compassionate and skilled Physical Therapist (PT) for a 13-week assignment in a Skilled Nursing Facility (SNF) setting. This opportunity is ideal for therapists who enjoy working with a geriatric patient population and are dedicated to improving mobility, independence, and overall quality of life through rehabilitative care.
Requirements
- Active Arizona Physical Therapist License required at time of submission
- Candidates who have already applied for Arizona licensure may also be considered if an estimated approval date can be provided
- Skilled Nursing Facility (SNF) or rehabilitation experience preferred
- Strong communication, interpersonal, and patient care skills
- Ability to work independently in a fast-paced clinical environment
- Strong organizational and documentation skills
Responsibilities
- Evaluate and treat patients in a Skilled Nursing Facility environment
- Develop individualized treatment plans focused on mobility, balance, strength, and functional independence
- Provide therapeutic interventions and rehabilitation services tailored to patient needs
- Collaborate with interdisciplinary healthcare teams to optimize patient outcomes
- Maintain timely, accurate, and compliant clinical documentation
- Educate patients and caregivers regarding treatment plans, safety, and home exercise programs
- Monitor patient progress and adjust treatment plans as appropriate
- Ensure compliance with therapy standards, facility policies, and healthcare regulations
