About The Position

The Physical Therapist provides home health care to patients in North County San Diego. The role evaluates patients’ physical conditions, develops individualized treatment plans, delivers therapeutic interventions, educates patients and caregivers, collaborates with an interdisciplinary team, and documents care and progress.

Requirements

• Active Physical Therapist license in the practicing state.
• Master’s or Doctorate degree in Physical Therapy.
• Minimum of 1 year of experience as a Physical Therapist.
• Current BLS and hands-on CPR certification.
• Valid driver’s license and reliable, insured vehicle.

Nice To Haves

• Home health experience is strongly preferred.

Responsibilities

• Evaluate and assess patients’ physical conditions and develop individualized treatment plans.
• Implement therapeutic exercises and techniques to relieve pain and improve mobility, strength, and coordination.
• Educate patients and caregivers on exercises, safety precautions, and self-care strategies.
• Collaborate with an interdisciplinary team to provide holistic, patient-centered care.
• Maintain accurate and timely documentation of patient progress and treatment plans.
