About The Position

The Physical Therapist evaluates patients’ physical conditions, develops individualized treatment plans, and delivers therapeutic exercises and techniques to relieve pain and improve mobility, strength, and coordination. The role includes educating patients and caregivers on exercises, safety precautions, and self-care strategies; collaborating with an interdisciplinary team to provide holistic, patient-centered care; and maintaining accurate, timely documentation of patient progress and treatment plans. The position is in home health in North County San Diego.

Requirements

• Active Physical Therapist license in the practicing state.
• Master’s or Doctorate degree in Physical Therapy.
• Minimum of 1 year of experience as a Physical Therapist.
• Current BLS and hands-on CPR certification.
• Valid driver’s license and reliable, insured vehicle.

Nice To Haves

• Home health experience is strongly preferred.
