About The Position

Las Colinas Post Acute is seeking a skilled and compassionate Registered Physical Therapist (PT) to join our rehabilitation team. This is an excellent opportunity to provide impactful, patient-centered care while working alongside a collaborative and supportive clinical team. Las Colinas Post Acute is a premier skilled nursing and rehabilitation facility serving San Bernardino County. We provide comprehensive short-term and long-term care, rehabilitation, and respite services in a safe and welcoming environment. Requirements

Active Physical Therapist (PT) license in the state of California
Graduate of an accredited Physical Therapy program
Strong commitment to patient-centered care
Excellent communication and teamwork skills

Nice To Haves

Experience in a skilled nursing or post-acute setting preferred

Responsibilities

Evaluate residents and develop individualized treatment plans
Provide therapeutic interventions to improve mobility, strength, balance, and functional independence
Collaborate with an interdisciplinary team to implement and adjust care plans
Supervise and support therapy assistants as appropriate
Accurately document evaluations, progress, and outcomes in compliance with regulatory standards
Deliver high-quality care with compassion, professionalism, and clinical excellence
