About The Position

Apollo Home Healthcare is hiring a full-time Physical Therapist to provide in-home care to patients in the Northern California Bay Area, within Alameda or Contra Costa County. The role focuses on helping patients regain independence, mobility, and daily functioning through physical therapy evaluations, treatments, and individualized plans of care. The Physical Therapist works independently in patients’ homes, collaborates with a broader healthcare team, and oversees Physical Therapy Assistants. A $5,000 hiring bonus is offered to full-time Physical Therapists, paid after six months of employment.

Requirements

Graduate of an accredited Physical Therapy program (Bachelor’s, Master’s, or DPT).
Active California Physical Therapist license.
CPR/BLS certification.
Strong communication and organizational skills.
Reliable transportation and willingness to travel to patients’ homes.
Comfort working independently in a home health setting.

Nice To Haves

Home health experience preferred.
Experience charting in Kinnser/Wellsky preferred, but not required.

Responsibilities

Provide in-home physical therapy evaluations and treatments.
Assess patients’ strength, mobility, balance, and ability to function safely at home.
Create and update individualized plans of care that support independence and recovery.
Help patients regain mobility and confidence in their home environment.
Educate patients and caregivers on exercises, safety, and fall prevention.
Document visits and evaluations in WellSky/Kinnser in a timely manner.
Collaborate with nurses, therapists, and the broader care team to support patient outcomes.
Supervise Physical Therapy Assistants and support their clinical development.
Participate in care planning, case conferences, and discharge planning.
